Purpose-built for every I/DD setting

ICF/IID Facilities

Active treatment, med passes, physician orders, and survey readiness.

HCBS Waiver Providers

Service documentation, ISPs, and outcomes across homes and community sites.

Day Programs

Attendance-friendly daily documentation tied directly to plan outcomes.

Vocational Services

Goal tracking and progress notes that follow each person, not the paperwork.

Everything your agency runs on. One platform.

No bolted-on modules, no third-party add-ons, no surprise invoices. Every capability below ships with EmpowerEHR.

eMAR that guides every pass

A color-coded medication grid your staff reads at a glance — scheduled, given, held, missed, and refused each get exactly one meaning. Vitals-based holds, PRN follow-ups, and inventory counts are built into the pass itself.

True CPOE, not a fax queue

Physicians and nurses enter medication and general orders against a standardized catalog with LOINC, SNOMED, and CPT codes. Verbal orders route for co-signature; standing orders generate their own recurring tasks.

Person-centered ISPs

Build Individual Service Plans with outcomes, supports, risk plans, and team signatures — then let daily documentation flow straight from the plan, so DSPs document against real goals, not free text.

Incident management built for compliance

MUI, UI, and general incident workflows with configurable required fields, witness statements, review chains, and closure lockdown — so nothing gets edited after the record is sealed.

Nursing delegation & competency

Track Ohio DODD nursing delegation, individual-specific training, competency checklists, and supervision plans — with expiration alerts before a lapse becomes a citation.

Flowsheets, vitals & results

Clinical flowsheets with threshold alerts, lab and imaging results with review workflows, and trends your nurses can actually see — abnormal values never hide in a PDF.

Security & compliance

HIPAA isn't a checkbox here. It's the foundation.

EmpowerEHR was architected for protected health information from the first line of code — not retrofitted after the fact.

  • Complete audit trail. Every view and every change of a clinical record is logged, immutable, and retained to HIPAA standards.
  • Role-based access with client scoping. Staff see only the individuals on their caseload and at their location — with a break-glass path for true emergencies, fully logged.
  • Multi-factor authentication. TOTP-based MFA with managed trust windows designed for shared nursing-station workstations.
  • True tenant isolation. Each agency's data lives in its own database schema — not commingled rows behind a filter.
  • Tamper-evident e-signatures. Cryptographically sealed signature capture for plans, orders, and consents — including remote signing for guardians and team members.

Built on proven, modern technology

EmpowerEHR runs on the same class of technology trusted by banks and government systems — engineered for reliability, security, and speed.

Standards-based coding

ICD-10, RxNorm, SNOMED CT, LOINC, and CPT built into diagnoses, medications, and orders.

Encrypted everywhere

TLS in transit, encryption at rest, and PHI-scrubbed operational logging.

Fast on any device

A responsive web app that works on the med-cart laptop, the office desktop, and the tablet in the van.

Always current

Continuous updates with zero-downtime deploys — no version upgrades to schedule, ever.

Still running your agency on software older than your newest DSP?

Most I/DD platforms were built in the 2000s and priced by the module. See how a clean-slate, all-inclusive EHR compares.

Pricing

One subscription. Everything included.

We don't do the module menu. Every capability on this site — eMAR, orders, ISPs, incidents, delegation, documents, reports — is in the base subscription, priced by the size of your agency. Implementation and training are part of the plan, not a surprise line item.

Frequently asked questions

Is EmpowerEHR HIPAA compliant?

Yes — and we can show you how, not just say it. Every access to a clinical record is logged, audit trails are immutable and retained to HIPAA standards, each agency's data lives in its own isolated database schema, and staff sign in with multi-factor authentication. We sign a Business Associate Agreement with every customer.

What does EmpowerEHR cost?

One all-inclusive subscription sized to your agency — no per-module pricing, and implementation and training are included. Contact us and we'll give you a straight answer with a real number for your size.

Can you migrate our data from our current system?

Yes. We run a guided migration for clients, medications, plans, and documents, and you validate everything in a review environment before go-live. Your team never starts from a blank database.

Do you support Ohio DODD requirements?

Ohio is built in by name: MUI/UI incident workflows, DODD nursing delegation with competency and supervision tracking, and the Ohio ISP with e-signatures and revision history. Ohio was our first jurisdiction, not our last — additional state compliance packs are in development. See our public roadmap for what's next, or tell us your state.

Does it work on tablets and phones?

EmpowerEHR is a responsive web application — it runs in the browser on the med-cart laptop, the office desktop, and the tablet in the community, with nothing to install or update.

What about Medicaid billing and EVV?

Both are on the active roadmap, and we'd rather tell you that plainly than sell you a checkbox. If billing is your first priority today, ask us about timelines on your demo call — we'll give you an honest answer.
